How to complete your repeat prescription request

Please read the guidance below carefully before submitting your request.

• This form is for existing ISC-CARE clients who are currently under active ADHD medication treatment with us.

• Please ensure you have had a medication review within the last 6 months before submitting your request.

Clinical observations (required)

You will be asked to provide your most recent clinical observations (OBS). Please ensure you have the following available:

• Blood pressure (systolic and diastolic)

• Heart rate (pulse)

• Weight (where requested)

Observations must be recent and accurate. Out-of-date or missing OBS may delay your prescription.

Completing the form

• Please complete all sections of the form fully and accurately.

• Ensure your personal details and medication details are correct.

• Read and complete all declaration and consent sections before submitting.

Payment

• The fee for a repeat prescription is £33.50 (maximum 28-day supply).

• Please ensure your payment is completed successfully at the end of the form.

• Your request will not be processed until payment has been received.

• Repeat prescription requests are non-refundable once submitted.

• Submitting incomplete or incorrect information may result in delays or the need to resubmit the form.
