Step 1 – Complete The Transfer Application
Patients complete our Transfer ADHD Care application form and provide information about their diagnosis, medication history and previous provider.
Step 2 – Clinical Review
Our ADHD clinical team reviews the available evidence and determines whether further information is required.
Step 3 – Treatment Planning
If appropriate, patients may be offered:
Ongoing medication monitoring

Step 4 – Ongoing ADHD Support
Patients remain under specialist ADHD supervision with regular reviews and clinical monitoring.

Acceptance of Shared Care always remains at the discretion of the individual GP practice.
