Adjustment Letters & Full Reports

At ISC-CARE, we recognise that following a neurodevelopmental assessment, many clients may need documentation to support conversations with employers, universities, schools, healthcare providers, or other organisations.

For this reason, we offer two types of documentation:

  • Adjustment Letters - £50

  • Full Clinical Reports - £125

These documents serve different purposes depending on the level of detail required.

Adjustment Letters

An Adjustment Letter is a short confirmation document stating that the client has undergone a formal assessment with our service and has received a diagnosis of either ADHD or Autism (ASD).

These letters are designed to provide simple confirmation of diagnosis, together with general information about the condition.

Adjustment letters:

  • Confirm that a formal assessment has taken place

  • Confirm that a diagnosis has been made

  • Provide general information about ADHD or autism

  • Include generic examples of reasonable adjustments

Importantly, adjustment letters do not contain personalised clinical findings and are not based on the detailed information gathered during the assessment.

Instead, they provide general guidance about how ADHD or autism may affect individuals in work, education, or daily functioning, which organisations can consider when discussing support options.

These letters are often sufficient for:

  • Employers or HR departments

  • Universities or colleges

  • Schools or SEN teams

  • Occupational health services

  • General confirmation requests

Full Adjustments Reports

A Full Adjustments Report is a more detailed document prepared for clients who require comprehensive documentation.

Unlike adjustment letters, full reports:

  • draw directly from the client’s assessment findings

  • explain how symptoms affect functioning

  • provide tailored recommendations for support or adjustments

  • include more detailed clinical context

These reports are typically requested where a more substantial professional document is required.

This may include:

  • workplace support planning

  • university disability services

  • educational support assessments

  • legal or occupational health contexts

Which Document Do I Need?

In many cases, organisations only require simple confirmation of diagnosis and general information about the condition, in which case an Adjustment Letter is usually sufficient.

Where more detailed documentation is required, a Full Adjustments Report may be more appropriate.

ADHD and Autism Documentation

Our documentation services are available for clients who have completed assessments for:

  • Attention Deficit Hyperactivity Disorder (ADHD)

  • Autism Spectrum Disorder (ASD)

Letters and reports will confirm the relevant diagnosis and provide appropriate supporting information.

All documentation provided by ISC-CARE confirms that a formal assessment has taken place.

Recommendations included within letters or reports are provided as guidance only, and any decisions regarding adjustments or support remain the responsibility of the relevant organisation.
